On the intake form, specify whether you have insurance or are self-pay.
If you are self-pay we will inform you of pricing prior to your appointment.
If you have insurance, please bring your insurance card to the appointment. You may also send us a photo of the front and back of your insurance card by emailing (secure@coberhernia.com), texting (561-556-2286), or uploading to the Carepatron portal.
On the intake form, please note your primary care provider if you have one.
We will contact them to obtain your last office visit note and any relevant labs and/or imaging.
For our out-of-town patients, or patients seeking a second opinion, we recommend having your last surgeon note and any imaging sent to us -- if you inform us of the office location / radiology center informatino we can request it on your behalf.

We usually have minimal to no wait times but we always recommend having a good book handy. There will no additional paperwork to fill out so we do not require patients to arrive very early.
During your appointment, the doctor will:
Conduct a history and physical examination.
Discuss treatment options and recommendations with you.
We also seek to accommodate any special requests you may have.
After the appointment, your treatment recommendations will be shared with you and your primary care doctor via our online portal.
